By signing this document, you are acknowledging you are the direct next of kin or authorized representative to the deceased and you are giving us permission to bring the deceased into our care as soon as they are released by the proper authorities.

After reviewing all information submitted to us, you may be asked to sign the following documents in-person or electronically.

- Statement of Funeral Goods & Services

- Decedent Positive Identification

- Biometric Data Authorization, if applicable

- Life Insurance Assignment or Claim, if applicable

- Cremation Authorization, if applicable
